The workshop provided the third annual update on the city's employee health and wellness center operated by CARE TC, located at City Hall Promenade. The presentation covered 2024 annual performance, 2025 year-to-date metrics, diabetes management outcomes, return on investment, and a proposal to add a nurse practitioner to address capacity constraints.
Presentation Highlights

- Return on Investment (ROI): For 2024: clinic costs ~$1.1M, total value ~$1.2M, ROI of 1.1. For 2025 (12-month rolling through Sept 2025): ROI 1.2. Components included clinic visits, PHAs, labs, prescriptions, RN outreach, productivity savings (~$243K in 2024, ~$270K in 2025).
- Proposal: Need for a part-time or full-time nurse practitioner to alleviate 91% capacity and improve appointment availability. Also consideration of space expansion but not immediate.

Okay. Hello, everyone. The mayor will not attend the meeting this evening, so I will be chairing as Vice Mayor. Madame Clark, please call the role. Good evening, Mayor, Vice Mayor, Commissioners, City Managers, City Attorney, City Clerk, Angelita DRS Deputy Director of Human Resources. The city contracts with CARETC to operate a full service primary care employee health and wellness center exclusively for City of Miramar employees, retirees, and their dependents. Located at 11606 City Hall Promenade, suite 203. The center offers early morning and evening hours to accommodate varying work schedules and is staffed by a full-time physician and two medical assistants. This is our third annual commission workshop to provide a status update on the center's utilization, performance, and its impact on employee health outcomes and the city's claims costs. We will also review recommendations and strategies to further strengthen the value of the center. Before we begin, I would like to introduce the members of the Kerry TC team joining joining us today. Aaron McDill, Vice President of Client Success, Damie Garcia Monsoon, Director of Client Success, and our very own Doctor of Osteopathic Medicine, Dr. Tiffany Sales. At this time, I would like to turn it over to Aaron McDill and the CARE TC team for our presentation. Good evening, everyone. Thank you all for having us and thank you for the introduction, Angelita. So my name's Erin McDill, Vice President of Client Success with CARE ATC, and we appreciate you all having us this evening. So let's go ahead and the presentation. All right. So we're gonna begin our workshop with the 2024 annual review. Also, we'll have a glimpse into 2025 to see where we are from year to date through September 30th as well. So we're gonna begin with our welcome, and then we'll have a moment that matters, dive into our health center review, our diabetes program review, review of the return on investment for both 24 and where we are currently with 2025, and then also a glimpse into 2025 to share where we are through January through September as well. So we like to start all of our meetings both internally and externally with what we call a moment that matters. And this is really to highlight why we do what we do, specifically why Dr. Sales does what she does. So we're gonna begin with a patient that saw Dr. Sales. Um, this was a 56-year-old female that had a past history of diabetes, hypertension, hyperlipidemia, and was also a smoker for 25 years. While Carrie TC has co-managed her condition, she also was seeing the uh endocrinologist as well, and as well as additional specialists for her preventative measures. While Dr. Sales was seeing her for a wellness exam, she found out that she was due for her preventatives but had not yet been scheduled. That being said, since she was already there at the health center for time and convenience, Dr. Sales went ahead and performed her wellness exam at the Carrie TC clinic. During the time that the patient was there, it was determined that she would need some additional screenings from lung cancer, breast cancer, cervical cancer, and osteoporosis screening based on her uh her age and the timing of her preventative visits. While she um was up to date on her colon cancer screening, her osteoporosis and cervical cancer screenings were negative, while her lung cancer screening did show that it was positive for a small nodule as well as a left breast nodular density. Her mammogram also found a calcified mass in her left breast. Following the evaluation, both revealed both breast cancer and lung cancer. While the direct communication between Dr. Sales, the specialist, oncologists, pulmonologists, and surgeons were supporting the patient. The cancers were promptly identified in treatment. And this is essentially why we do what we do. With the early diagnosis and intervention that Dr. Sales provides, it helped to provide this patient the support and the care that they did need. So thank you, Dr. Sales. So we'll begin looking at the executive summary for 2024. Ultimately, this is a slide with the data that shows the summary of all of the additional data points that we're going to discuss and dive into further detail about. 84% of patients with chronic conditions, meaning that 84% of the population that is utilizing the health center has multiple chronic conditions that Dr. Sales is managing.
